Registration and Health Forms
Below are the links for the Camper Data Form and the Health Form required for all campers coming to Camp Horizon 2011. Please submit the Camper Data Form no later than JULY 18th to secure a spot for your camper. The Health Form should be brought to Camp Horizon on Saturday, August 27th.
Both files are in Microsoft Word format. If you need a different format, please send an email to Steven: steven@camp-horizon.com
